Site-Specific Histopathology Forms

The histopathology forms here are based on protocols published by the Royal College of Pathologists. The content and format of the forms have been agreed by the Yorkshire Histopathology Network and in some cases contain items additional to those recommended by the Royal College.

NYCRIS can supply further copies of the forms on request to each Trust. Alternatively, these can be downloaded (below) for local printing. For ease of recognition, each form is printed on a different colour paper, as shown below.

NYCRIS downloadable reports (Adobe PDF Reader required) have been designed to print clearly in colour or in black & white. When printing, please select 'no page scaling' in the print dialog to retain maximum size, and print double-sided to see reports as intended
